Every month in 2013, we will have a drawing which will be held on the first week of each month for the previous month entries) Fill out the ticket and drop it into a box near the prescription counter. Please put your name and phone number on the ticket. If you didn’t receive your entry form, stop by the store today and pickup your entry forms for the monthly drawing.

Rules for monthly drawing:
1. Store employees or family members are not eligible to participate.
2. Prizes, if possible, will be displayed in the front of the store to be viewed by the customer during the month.
3. NO prescription is required to enter the monthly drawings.
5. Prizes CAN NOT be exchanged for another item/prize or monetary exchange.
